2020 1095-B Forms

City of Cleveland employees and former employees who worked in 2020 (and were eligible for city health coverage) can obtain their 2020 1095-B form upon request from the HR department by emailing jjeude@clevelandtexas.com or by mailing a request to City Hall, Attn: J. Jeude, 907 E Houston, Cleveland TX 77327. Questions you may contact HR at 281 592 2667. The HR department has 30 days from date of request to provide the form. This form is no longer needed to file your taxes.
